Wegovy is the brand name for semaglutide at the FDA-approved weight-loss dose — 2.4 mg once weekly via subcutaneous injection. Manufactured by Novo Nordisk, FDA-approved June 2021 for chronic weight management in adults with obesity (BMI ≥30) or overweight (BMI ≥27 with at least one weight-related condition). Same active ingredient as Ozempic, different label and dose schedule.

Quick definition

Wegovy is the on-label weight-loss version of semaglutide. Ozempic is the diabetes version. The drug molecule is identical. The package and dose titration are different.

How it actually works

Same mechanism as all GLP-1 agonists: slowed gastric emptying, central appetite suppression, improved insulin response. The 2.4 mg weekly dose was chosen in the STEP trial program as the dose where benefits maximize and side effects plateau.

The pivotal STEP 1 trial (Wilding et al., 2021, NEJM, n=1,961) randomized adults with BMI ≥30 (or ≥27 with comorbidity) to 68 weeks of weekly Wegovy or placebo. Average weight loss: 14.9 percent on Wegovy versus 2.4 percent on placebo. Roughly half of Wegovy participants lost ≥15 percent of bodyweight — a result previously only seen with bariatric surgery.

Dose titration runs over 16 weeks: 0.25 mg → 0.5 → 1.0 → 1.7 → 2.4 mg. Going faster increases nausea and discontinuation. Going slower stretches out the time to therapeutic dose. Most prescribers stick to the label schedule.

Why it matters for weight loss

If you qualify by BMI and your insurance covers it, Wegovy is the most-evidenced non-surgical fat-loss tool available. Out-of-pocket cost without insurance runs $1,300 to $1,500 per month. Many employer plans cover it; many do not. Medicare currently does not cover GLP-1s for weight loss (only for diabetes).

Common misconceptions

The biggest myth: Wegovy is for cosmetic weight loss. It's an FDA-approved obesity medication for medically defined obesity. If your BMI is 24, no legitimate clinician should be prescribing it.

The second myth: you can stop at goal weight and keep the loss. The STEP 4 extension trial showed regain of about two-thirds of lost weight within a year of stopping. The implication: this is a chronic-disease medication, not a 6-month protocol.
